SUMMARY: To clarify the path of the temporal branch of facial nerve (TB) crossing the zygomatic arch (ZA). Eighteen fresh adult heads specimens were carefully dissected in the zygomatic region, with the location of TB as well as its number documented. The hierarchical relationship between the temporal branch and the soft tissue in this region was observed on 64 P45 plastinated slices. 1. TB crosses the ZA as type I (21.8 %), type II (50.0 %,), and type III (28.1 %) twigs. 2. At the level of the superior edge of the ZA, the average distance between the anterior trunk of TB and the anterior part of the auricle is 36.36±6.56 mm, for the posterior trunk is 25.59±5.29 mm. At the level of the inferior edge of the ZA, the average distance between the anterior trunk of TB and the anterior part of the auricle is 25.77±6.19 mm, for the posterior trunk is 19.16±4.71 mm. 3. The average length of ZA is 62.06±5.36 mm. TB crosses the inferior edge of the ZA at an average of 14.67±6.45 mm. TB crosses the superior edge of the ZA at an average of 9.08±4.54 mm. 4. At the level of the ZA, TB passes on the surface of the pericranium while below the SMAS. The TB obliquely crosses the middle 1/3 part of the superior margin of the ZA and the junction of the middle 1/3 part and the posterior 1/3 part of the inferior margin of the ZA below the SMAS while beyond the periosteum. It is suggested that this area should be avoided in clinical operation to avoid the injury of TB.


El objetivo de estudio fue esclarecer el trayecto del ramo temporal del nervio facial (RT) que cruza el arco cigomático (AC). Se disecaron la región cigomática de 18 especímenes de cabezas sin fijar de individuos adultas y se documentó la ubicación del RT y su número de ramos. La relación jerárquica entre el ramo temporal y el tejido blando en esta región se observó en 64 cortes plastinados o P45. 1º El RT cruza el AC como tipo I (21,8 %), tipo II (50,0 %) y tipo III (28,1 %). 2º A nivel del margen superior del AC, la distancia promedio entre el tronco anterior de RT y la parte anterior de la aurícula fue de 36,36±6,56 mm, para el tronco posterior fue de 25,59±5,29 mm. A nivel del margen inferior del AC, la distancia promedio entre el tronco anterior del RT y la parte anterior de la aurícula era de 25,77±6,19 mm, para el tronco posterior era de 19,16±4,71 mm. 3º La longitud media de RT fue de 62,06±5,36 mm. EL RT cruzaba el margen inferior del AC a una distancia media de 14,67±6,45 mm. El RT cruzaba el margen superior del AC a una distancia media de 9,08±4,54 mm. 4º Anivel del AC, el RT pasaba por la superficie del pericráneo mientras se encuentra por debajo del SMAS. El RT cruza oblicuamente el tercio medio del margen superior del AC y la unión del tercio medio y el tercio posterior del margen inferior del AC por debajo del SMAS, más allá del periostio. Se sugiere que esta área debe evitarse en la operación clínica para evitar la lesión de la RT.

Objective@#To investigate the impact of physical self-efficacy on physical activity and physical health among university students, so as to provide insights into formulation of the strategy to improve physical activity and physical health among university students.@*Methods@#Freshmen and sophomores were sampled from a university in Guangzhou City using a convenience sampling method from December 2021 to April 2022. Students' demographics and types of sport exercises were collected using questionnaire surveys. Physical self-efficacy was tested using the Physical Self-efficacy Scale, and physical activity was measured using the International Physical Activity Questionnaire-Short, while physical health was evaluated using the school physical health standard test. The associations of physical self-efficacy with physical activity and physical health were examined using a multivariable logistic regression model. @*Results@#Totally 4 171 questionnaires were allocated, and 3 811 valid questionnaires were recovered, with a effective recovery rate of 91.37%. The respondents included 1 582 males (41.51%) and 2 229 females (58.49%), and included 1 967 freshmen (51.61%) and 1 844 sophomores (48.39%). The median score of physical self-efficacy was 36 (interquartile range, 7) points, and there were 1 777 students reaching the national standard of physical activity (46.63%) and 1 112 students with excellent and good physical health (29.18%). Multivariable logistic regression analysis showed that physical self-efficacy was a promoting factor for the proportion of reaching the national standard of physical activity (OR=1.054, 95%CI: 1.043-1.064) and excellent and good physical health (OR=1.109, 95%CI: 1.096-1.122) after adjustment for gender, grade, specialty and source of students. @*Conclusion@#The improvement of physical self-efficacy may increase the proportion of reaching the national standard of physical activity and excellent and good physical health among university students.

Objective@#To explore the relationships among social support, depression, anxiety and suicide ideation among middle school students in rural areas of south Sichuan Province so as to provide a scientific basis for promoting middle school students’ mental health and reducing suicide ideation.@*Methods@#A sum of 2 217 students of grade 7 and 10 from 2 township middle schools in mountainous areas of Zizhong County, Sichuan Province, were investigated by means of typical sampling and cluster random sampling. Social Support Rating Scale (SSRS), Self-rating Scale for Depression (SDS), Self-rating Anxiety Scale (SAS) and Self-compiled adolescent health questionnaire were used to measure suicidal ideation, and structural equation model was used for statistical analysis.@*Results@#The depression score of middle school students was (13.51±12.57), and the self-rating scale for Depression score of girls was (14.61±12.67), higher than that of boys (12.24±12.34). The SSRS score of Social Support Rating Scale of junior high school students was (35.49±5.88), which was higher than that of senior high school students (34.70±5.43), and the differences were of statistical significance(t=4.46, 2.67, P<0.05). The results of structural equation model showed that depression and anxiety directly affected suicidal ideation, and the standardized effects were respectively 0.11 (95%CI=0.06-0.16) and 0.09(95%CI=0.05-0.14). Depression and anxiety were the intermediate variables in the relationship between social support and suicidal ideation, and the mediating effects were respectively -0.46(95%CI=-0.51--0.41) and -0.21(95%CI=-0.27--0.16). Studying period could play a mediating role in the indirect effect made by social support on suicide ideation through anxiety.@*Conclusion@#Anxiety and depression have significant mediating effect on the relationship between social support and suicidal ideation among middle school students in rural areas of south Sichuan province. Anxiety and depression intervention are important measures to reduce suicide ideation among low social supporters.

Morus alba L., known as white mulberry, is a medicinal plant belongs to family Moraceae. It has long been used commonly in Ayurvedic for the treatment of lung-heat, cough, asthma, hematemesis, dropsy and hypertension. In the present study, seven prenylated flavonoids, along with four benzofuran compounds were isolated by means of repeated column chromatography. The structures of the known compounds were identified as kuwanon G (1), kuwanon E (2), kuwanon T (3), morusin (4), sanggenon A (5), sanggenon M (6), sanggenol A (7), moracin R (8), mulberofuran G (9), mulberofuran A (10) and mulberofuran B (11), by comparing their spectroscopic data with those reported in the literature. Objective @#To compare the stress distribution of residual posterior root restored with a new titanium alloy post of different diameter and taper, and discuss the rational design of posterior tooth post by the three-dimensional finite element method. @*Methods @#Mandibular second premolar image data was obtained by CBCT, and the mandibular second premolar post models with different diameter and taper were established with the help of Mimics data conversion, Geomagic Studio image processing technology and Creo/Parametric software. Then Abaqus was used to simulate the load condition of prosthesis and analyze the Von Mises stress of the cervical and apical regions.@*Results@#With the same diameter, the influence of the different post tapers on the peak stress of the root cervical area was irregular. When the taper was the same, the peak stress of the root apex area decreased firstly and then increased as the diameter increased. When the diameter was 1 mm, the peak stress of the root cervical and apex area was the lowest.@*Conclusion @#The effect of diameter on the stress distribution of dentin is more important than that of taper. To select the reasonable diameter and taper of the post is helpful to reduce the peak stress of the root cervical and apex area.

@#<p><strong>OBJECTIVES:</strong> An inter-arm difference in systolic blood pressure (IADSBP) of 10 mmHg or more has been associated with cardiovascular disease (CVD) and increased mortality in T2DM patients. We aim to study ethnic disparity in IADSBP and its determinants in a multi-ethnic T2DM Asian cohort.<br /><strong>METHODOLOGY:</strong> Bilateral blood pressures were collected sequentially in Chinese (n=654), Malays (n=266) and Indians (n=313). IADSBP was analyzed as categories (<br /><strong>RESULTS:</strong> Malays (27.4%) and Indians (22.4%) had higher prevalence of IADSBP ?10 mmHg than Chinese (17.4%) (p=0.002). After adjustment for age, gender, duration of diabetes, hemoglobin A1c, body mass index (BMI), heart rate, pulse wave velocity (PWV), estimated glomerular filtration rate (eGFR), albumin-to-creatinine ratio (ACR), smoking, hypertension, soluble receptor for advanced glycation end products (sRAGE), and usage of hypertension medications, ethnicity remained associated with IADSBP. While Malays were more likely to have IADSBP ?10 mmHg than Chinese (OR=1.648, 95%CI: 1.138-2.400, p=0.009), Indians had comparable odds with the Chinese. BMI (OR=1.054, 95%CI: 1.022-1.087, p=0.001) and hypertension (OR=2.529, 95%CI: 1.811-3.533, p<0.001) were also associated with IADSBP ?10 mmHg.<br /><strong>CONCLUSION:</strong> IADSBP in Malays were more likely to be ?10 mmHg than the Chinese which may explain their higher risk for CVD and mortality. Measuring bilateral blood pressures may identify high-risk T2DM individuals for intensive risk factor-management.</p>

OBJECTIVE: To test the effectiveness of the METS (modified extension-type sling) in reducing subluxation in patients with hemiplegic shoulders, and to assess the functional support of METS as the angle of the elbow changes. METHOD: Sixteen flaccid hemiplegics were assessed by the anteroposterior shoulder radiographs. Three parameters, vertical distance (VD), horizontal distance (HD) and joint distance (JD), were measured on both affected and unaffected sides before and after the fitting of the METS and the Bobath sling. Three parameters were also measured in each elbow flexion angle (0degrees, 30degrees, 60degrees, 90degrees) after the fitting of the METS and extension-type sling with a forearm cuff to evaluate the effectiveness in the dynamic condition. RESULTS: In the Bobath sling, the mean values of VD and JD reduced significantly (p0.05). In the Bobath sling, VD, HD and JD were different from the unaffected side (p0.05). As the elbow flexion angle increased with the METS, three parameters were not different from the unaffected side (p>0.05), but with the extension-type sling with a forearm cuff, VD and JD were different from the unaffected side (p<0.05). CONCLUSION: The METS was more effective not only in the reduction of the hemiplegic shoulder subluxation, but also in the maintenance of reduction as the elbow flexion angle changes within various activities.

RESUMO

Spontaneous rupture of the bladder is usually associated with a history of recent trauma, instrument injury, or existing bladder disease. However, there are also occasional reports of bladder rupture associated with pregnancy, most of which are complications of cesarean section. However, spontaneous bladder rupture following normal vaginal delivery is very rare. Patients with bladder rupture need a rapid, sensitive diagnosis and treatment because of a high risk of mortality and morbidity. We report a case of bladder rupture after normal vaginal delivery, which was successfully managed. A patient with small bladder rupture and ascites after normal vaginal delivery was treated by urethral catheterization and use of antibiotics, without operation.

OBJECTIVE: To introduce a Modified triangular Bobath sling (MTBS) and a MTBS with distal support (MTBSDS) as a new slings and to compare with previous slings in the therapeutic effects of shoulder subluxation in stroke patient. METHOD: Using a simple X-ray with and without slings, the vertical and horizontal distances were measured for 17 stroke patients. MTBS and MTBSDS were compared with 3 conventional slings, which were hemisling, Bobath sling and Extension-type sling, in the effects of correction. The questionaire was used to evaluate preference of patients. RESULTS: In two developed slings, inferior subluxation showed significant correction and lateral displace was less increased than a Bobath sling although it was not statistically significant (p<0.05). MTBS had the preference during non-walking because of comfort and easiness for donning and doffing, whereas MTBSDS had the preference during walking because of reciprocal arm swing and protecting the hemiplegic upper extremity from trauma. CONCLUSION: The results indicated MTBS and MTBSDS were more effective on shoulder subluxation in stroke patients. Further study on the development of slings for decrement of lateral displacement is recommended.

Endometriosis is defined as an extrauterine growth of endometrial tissue and it is primarily limited in the pelvis but it can also occur in the pleural cavity as well as pulmonary parenchyme. The diagnosis of pulmonary endometriosis is usually based on the clinical history of recurrent hemoptysis in association with menstrual cycle and by histopathologic confirmation of endometrial tissue in the lung parenchyme. Pulmonary endometriosis was first reported by Lattes in 1956, and dozens of cases have been reported so far. We experienced a case of 25 year old single woman with a history of hemoptysis in association with her menstruation. The bleeding focus was localized with chest CT scan and repeated fibrooptic bronchoscopy and basal segmentectomy of the right lower lobe was performed. The resected specimen shows endometrial stroma and glands of early proliferative phase with respiratory epithelium on the laterobasal bronchus. Her postoperative course was uneventful with no recurrence of hemoptysis during 6 months of follow-up in the outpatient clinic.

A retrospective analysis of 108 patients who had undergone transurethral prostatectomy for benign prostatic hyperplasia between January 1988 and December 1992 was performed to evaluate risk factors influencing intraoperative and postoperative morbidity and mortality. Of 108 patients complications occurred in 32 cases, with the morbidity rate of 29.6% but no death occurred. The most common postoperative complication was bleeding in 9 cases(8.3%) followed by incontinence in 7 cases(6.5%), capsular perforation in 4 cases(3.7%), failure to void in 4 cases(3.7%) and urethral stricture in 4 cases(3.7%). Risk factors which increased the morbidity of transurethral prostatectomy were age greater than 75 years and the presence of associated medical disease(p<0.05) but a resection time of more than 90 minutes, weight of resected tissue more than 30 gram and amounts of irrigating solution of more than 20 L did not increase the postoperative morbidity significantly. In conclusion, meticulous preoperative and postoperative cares are necessary because poor general condition increase the postoperative complications and long-term, prospective randomized studies are required to evaluate risk factors influencing postoperative morbidity after transurethral prostatectomy.

Progressive ischemia can result in gangrenous changes of the tissues. Most commonly the digits are affected initially, but progression to the hand is not unusual. Small amounts of infection superimposed on a severe chronic ischemic state can progress very rapidly to gangrene. Clinically, dry and wet gangrene should be differentiated. Dry gangrene represents mummification of tissue, and active purulent tissue and cellulitis are absent. Wet gangrene is characterized by active infection with cellulitis and purulent tissue planes and is an indication for urgent amputation to prevent ascending infection. We experienced a case of dry gangrenous extremity due to septic shock after anethesia.

RESUMO

Primary teratomas of the bladder are exceedingly rare, only several cases have been reported in the literature. A case of primary teratoma of the bladder in a 31 years old woman is presented and reported with a brief review of the literature. The patient was managed by segmental resection of the bladder with good results.
